Abstract

Considerations for improvement of soft tissue around implants are the width of keratinized gingiva and the thickness of surrounding soft tissue. There has been a lot of controversy over the necessity of the proper keratinized gingival width in implants to maintain periodontal health. However, when enough keratinized gingiva exists around implants, it is more esthetic, becomes more resistant to gingival recession or periodontal attachment loss and has an effect of reducing inflammation and discomfort in managing oral hygiene clinically. Therefore, the keratinized gingiva is clinically and biologically valuable even though the necessity of it is still a controversial issue scientifically. Through 3 clinical cases, apically positioned flap with autogenous tissue graft showed result of increasing keratinized gingiva around implants and deepening vestibular depth. Furthermore, attained keratinized gingiva showed 17 ~ 2% contraction after 10 ~ 13 months, but it still maintained without any complications. Therefore, such surgical approach in implant surgery is suggested as predictable method as it is also used in natural teeth for increasing keratinized tissue around implants and deepening vestibular depth.
